What this website collects
The appointment request form collects your name, contact phone, optional email, consultation and visit preference, preferred date and consent. The contact form also collects an administrative question and its category. Do not include medical history, reports, prescriptions or other clinical information.
Why it is used
The practice uses these details to respond, arrange or change appointments, coordinate professional enquiries and operate the website. Submitting a request is not a confirmed booking. No online payments or patient clinical records are collected by the new website forms.
Storage and access
Administrative requests are stored in this site’s WordPress database on its Hostinger-hosted VPS. Access is restricted to authorised practice roles. Content editors do not receive administrative-request access. Essential service logs and protected backups support operation and recovery.
Retention
New administrative requests are configured for contact-data anonymisation after 90 days. This is checked by a daily WordPress scheduled task when the site is running. Existing legacy forms and backups have separate retention arrangements; contact the practice for details.
Cookies and external services
Essential cookies support request receipts and WordPress staff sign-in. Public pages do not embed advertising pixels, video players or third-party maps. Google Maps opens only when you choose its link. Online consultations use Google Meet or Zoom under arrangements confirmed privately, subject to the selected provider’s privacy information.
The existing optional administrative chat assistant is not a clinical service. Do not send medical or sensitive information to an automated assistant. Contact the practice for information about the current chat provider.
Your choices and questions
You may ask the practice about access to, correction of, or deletion of your administrative information. Requests are reviewed with identity and any applicable record obligations in mind. Use the Privacy category on the contact form, without sending sensitive identity documents publicly.
